Egg donation
Egg donation provides an opportunity for women who cannot use their own cells to become pregnant.
This process allows in vitro fertilization with donated eggs, thus increasing the chances of a successful outcome.
The upper age limit for a woman who receives eggs in Serbia is 50.

What is egg donation?
Egg donation, that is, in vitro fertilization with donated eggs, is a method of infertility treatment when a woman under the age of 35 donates her eggs to enable another woman to have offspring.
The donated eggs are fertilized with the partner's sperm in an IVF laboratory and then the embryo is placed in the uterus where the pregnancy develops.
Embryos that meet the quality are frozen.
The egg donation process at the Gyn-Fertility Clinic Milenković
The process of preparing and managing the donation is carried out in the Clinic, while the procedure of in vitro fertilization with donated egg cells is carried out in cooperation with the partner hospital.
Pre-frozen oocytes are ordered from Ovobank in Spain.
I. Consultation and selection of eggs
The woman is examined in the same way as before any other infertility treatment:
- conversation;
- gynecological examination;
- transvaginal ultrasound;
- hormone analysis: anti-Müllerian hormone (AMH), thyroid-stimulating hormone (TSH) and, if necessary, other hormones
- by PCR method cervical swab for chlamydia and gonococcus
- blood tests for hepatitis B, hepatitis C, human immunodeficiency virus (HIV) and syphilis.
The male partner should do:
- semen analysis;
- PCR urethral swab for chlamydia or PCR analysis for chlamydia from urine;
- blood tests for hepatitis B, hepatitis C, human immunodeficiency virus (HIV) and syphilis;
The couple receives important information about the donation process related to the medical, legal, emotional and ethical aspects. Risk factors in pregnancy are discussed.
If egg donation is financed by the Republic Health Insurance Fund, additional analyses are required (see egg donation at the expense of Republic Fund of Health Insurance).
The couple talks to a psychologist, a lawyer and a gynecologist and gets important information about the egg donation process.
At the same time, a psychological assessment is done.
After that, the egg donor is selected.
- The egg donor is chosen so that the color of the skin, eyes and hair, height and body mass match as much as possible to the woman who needs the eggs.
- It is also possible to get information about education and hobbies (sports, music), as well as which country the egg donor is from.
- After selecting the characteristics, a query is sent to the egg bank.
- When the answer is received, the couple comes to a new consultation where they are informed about the characteristics of the donor and then they can accept or not accept the offer.
II. Details about the egg donation process
Egg donation is done in:
- women with Turner syndrome;
- women in premature menopause;
- women older than 43-44 years;
- women who have a genetic disease, and it is not possible to perform a pre-implantation genetic analysis;
- after several unsuccessful IVF procedures;
- in women who have had their ovaries removed;
- in women who had chemotherapy and radiation that led to cessation of ovarian function.
Donors go through a detailed medical and psychological evaluation.
The eggs are frozen and transported to the Clinic.
The couple chooses the appropriate package of donated eggs.
Three packages can be chosen:
- 3-4 eggs;
- 6-7 eggs;
- 8-9 eggs.
The packages are for couples who are self-funding IVF with donated eggs.
*Donation of egg cells through Republic Fund of Health Insurance
Republic Fund of Health Insurance covers the costs of the procedure with 6 donated eggs, which means that one transfer of embryos obtained after thawing the eggs and possibly another transfer if there are frozen embryos can be performed.
The application is made through eUprava, and the Commission makes a decision.
IVF at the expense of Republic Fund of Health Insurance is possible until the age of 45.
- Before thawing the eggs, it is necessary to prepare the endometrium as when transferring frozen embryos.
- The endometrium is prepared in a spontaneous or in hormone replacement treatment (HRT).
Spontaneous cycle – follows the natural cycle of ovulation.
HRT cycle - hormone therapy with estrogen and progesterone is used.
HRT protocol after IVF with donated eggs is most common used protocol.
- A woman comes for the first ultrasound check on the tenth day of the cycle, regardless of the type of protocol.

- The thickness and appearance of the endometrium is analyzed during the ultrasound examination.
- When the embryo transfer is done in a spontaneous cycle, the leading follicle is also monitored, and then it is necessary to analyze the luteinizing hormone for on day 2-4.
- The day of ovulation is determined, after which the eggs are thawed.
- The partner gives a sperm sample and fertilization is done in an IVF laboratory.
Depending on the development of the embryo, the embryo transfer is done on the second, third or on the fifth day.
- The woman comes for the first ultrasound check on the tenth day of the cycle in which the transfer will take place
- the eggs are thawed when the endometrium reaches the appropriate thickness and appearance, after preparation with estrogen;
- On the same day, the patient starts using vaginal progesterone;
- The partner provides a sperm sample, and the eggs are fertilized in an IVF laboratory.
Depending on the development of the embryo, the embryo transfer is performed on the second, third or fifth day.
- Embryo transfer is done with one, possibly two embryos.
- The remaining embryos that meet the criteria are frozen for future attempts.
12-14 days after the embryo transfer, a beta hCG test for pregnancy is performed i.e. beta hCG from urine or blood.
Criteria that must be met by an egg donor
The donor of egg cells is a psycho-physically healthy person, aged 21-35, without hereditary diseases in the family.
After a medical examination and filling out a detailed questionnaire about her health, the donor has an interview with a psychologist, where a psychological assessment of the donor (among other things, motives for donating) and so-called psycho-education about donating is carried out.
The donor also learns about the legal system of the country where the spermatozoa are donated.
In addition to the karyotype, i.e., chromosome analysis, genetic analyses are performed for egg donors for:
- cystic fibrosis;
- spinal muscular atrophy;
- deafness;
The donor is tested for hepatitis B, hepatitis C, human immunodeficiency virus (HIV), syphilis and chlamydia infections 30 days before the planned stimulation procedure and 7 days after egg aspiration.

Who are egg donors?
The egg donor is a psychophysically healthy person, 21-35 years old, with no hereditary diseases in the family. Donors are tested for genetic diseases as well as hepatitis B, hepatitis C, human immunodeficiency virus (HIV), syphilis and chlamydia infections 30 days before the planned stimulation procedure and 7 days after egg aspiration.
What is the upper age limit for egg donation in Serbia?
Egg donation in Serbia is allowed until the age of 50.
Do egg donors receive monetary compensation?
In most European countries, the law allows monetary compensation for time, absence from work and possible additional expenses. This compensation amounts to 500-1000 EUR. In the USA, female donors are paid and this amount is around 5000 US dollars.
Is the procedure anonymous?
Egg donation in Serbia is anonymous. A couple who receives an egg cell and a child born after the IVF procedure with a donated egg cell does not have the right to know the identity of the donor. In some countries, the procedure is not anonymous and the child has the right to know the identity of the donor when he/she turns 18, if he/she wishes.
What costs does the Republic Fund of Health Insurance cover?
Republic Fund of Health Insurance covers all the costs of transporting 6 eggs from abroad and the procedures that can be done with those cells. In practice, this means that after thawing the eggs and the fertilization, one transfer is done with a fresh embryo and possibly another one with frozen embryos, if there are any. All procedures at the expense of the RFZO are performed until the woman reaches the age of 45. For example, if the embryo from the donation process is frozen before the age of 45, and the embryo transfer of the frozen embryo is done after the age of 45, the Republic Fund of Health Insurance does not cover the costs of the frozen embryo.
How many eggs does a donor usually give?
If the procedure is financed by the Republic Fund of Health Insurance, one couple receives 6 frozen eggs. In self-funded patients, one couple receives 3-9 eggs. The chance of success increases with the number of eggs.
Is there a risk that the eggs will not survive the thaw?
The risk of eggs not surviving thawing is 5-30%, depending on the quality of the eggs, as well as the technique of freezing and thawing the eggs.
Is there a risk of transmitting genetic diseases from the donor?
The egg donor undergoes a detailed medical evaluation, analyses all chromosomes (karyotype) and is tested for cystic fibrosis, spinal muscular atrophy and deafness. The risk for a genetic disease cannot be completely ruled out, just as it cannot be ruled out even after a spontaneous pregnancy.
Is it possible to freeze excess embryos for later use?
All embryos that meet the quality are frozen, provided that the woman receiving the eggs gives written consent.
What is the success rate of IVF with donated eggs?
The expected success rate after IVF with donated eggs per transfer is around 50-60%, but there are large variations from country to country and from clinic to clinic.
How many attempts are recommended with donated eggs?
The number of IVF attempts with donated eggs is not limited and the assessment is individual. Republic Fund of Health Insurance finances one fresh embryo transfer and transfers of frozen embryos if there are any. The number of attempts for self-financing couples depends on economic possibilities, the age of the woman and the health condition of both partners. The upper limit for the egg donation process in Serbia is 50 years.
Is the pregnancy after IVF with donated eggs risky?
Pregnancy after IVF with donated eggs carries an increased risk of:
- bleeding after childbirth and the need for blood transfusion;
- premature birth:
- high blood pressure and preeclampsia in pregnancy;
- diabetes during pregnancy.
A twin pregnancy further increases the risk for pregnancy complications.
Can IVF with donated eggs be done with fresh eggs?
IVF with donated eggs can be done with fresh oocytes and then it is necessary to synchronize the stimulation of the donor with the preparation of the endometrium of the woman receiving the oocytes. Donation with fresh egg cells is currently not done in Serbia, although it is allowed by law.
For how many couples can one donor donate eggs?
According to the law in Serbia one donor can donate eggs for one couple. In most countries where donation is done, one donor can donate eggs for 4-6 couples.
Can a donor donate eggs to a person they know?
A donor can only donate eggs to a person they know in countries where donation is not anonymous. The donor must go through the same process of evaluation and tests as when donating to an unknown woman.
Should the parents tell the child that the pregnancy resulted from the donation procedure?
Whether the parents will inform the child about the donation procedure depends on the parents and it differs a lot from country to country, that is, the mentality of the environment in which they live. The advantage of being informed is to eliminate the minimal risk that the child will later be in a relationship with a blood relative.
Can I choose the characteristics of the donor (eye color, height, education)?
The woman receiving the eggs can choose the eye color, height and other physical characteristics as well as the education and hobbies (music, sports). After filling out the form, the inquiry is sent to the Egg bank, which selects the donor according to the required characteristics. Sometimes not all conditions are met, so the woman or the couple decides for themselves whether to accept the procedure with the available characteristics of the donor.
What are the benefits of the egg donation process?
The advantage of IVF with donated eggs compared to IVF with own oocytes is only in women who have a clear indication:
- in women with Turner syndrome;
- in women in premature menopause;
- in women older than 43-44 years;
- in women who have a genetic disease, and it is not possible to perform a pre-implantation genetic analysis;
- after several failed IV procedures;
- in women who have had their ovaries removed.
- in women who had cytostatic therapy and radiation that led to cessation of ovarian function.
Is there a risk that the descendants of a donor will meet later in life and start a family?
The risk of one donor's offspring becoming partners is very small, even in countries where one donor can donate eggs to 6 couples. In order to further reduce that risk, in some countries (Sweden, Great Britain, Switzerland) the donation is not anonymous, that is, children at the age of 18 can find out the identity of the donor.
Is there any risk for the donor?
An egg donor has a risk of hyper stimulation syndrome (which is very rare today). The risk of bleeding and infection after egg aspiration is less than 1%. Donor women go through a process of psycho-education where emotional, legal and ethical aspects are analyzed. One donor can donate eggs more than once and this will not reduce her ovarian reserve, nor will she enter menopause earlier.
